The story opens in the post-apocalyptic year of 2070, where a lone and immortal gunman named Cowboy roams the desolate American West. His solitary existence is disrupted when he encounters Mary Ann, a young woman who has just emerged from a nuclear fallout shelter, completely unaware of the devastated world outside. Cowboy, a survivor of the nuclear attacks who has inexplicably lived for over 200 years and cannot be killed, is initially seeking supplies in Mary's deserted town. He finds little of value, only water, and is about to leave when Mary, driven by curiosity and a desire to escape her confinement, asks to accompany him. He reluctantly agrees, and their journey begins.

Their travels lead them to a sign suggesting safety by following railroad tracks north. Despite Cowboy's inherent suspicion and warnings about the dangers of the mutated world, Mary's curiosity convinces them to investigate. Their path is soon violently interrupted by a grotesquely mutated wolf with a horrifying, secondary mouth in its neck. In the ensuing fight, Cowboy sustains a wound that miraculously heals almost instantly, confirming his near immortality to Mary. After defeating the creature and making a meal of it, they approach a walled settlement, leaving the reader to wonder if this town represents true sanctuary or a new set of dangers.
