Kotoko was finally able to share a lovey-dovey birthday with Naoki for the first time. While she's up in the clouds, a supermodel called Nobuhiro comes to the hospital, and everyone is ecstatic. Kotoko tags along to see him, but Nobuhiro turns out to be the little Nobu she knew years ago! Sadly, he's no longer the little angel she knew, and Naoki tries to convince Nobu to have an operation.
