Changes are in store for the devoted staff at the Good Karma Hospital, an underfunded clinic in coastal Kerala, India. British ex-pat Ruby Walker (Amrita Acharia, Game of Thrones) is still reeling following the abrupt departure of Gabriel (James Krishna Floyd, No Man's Land), her colleague and former flame. Then she's dealt another blow when her father is diagnosed with cancer. Meanwhile, two new doctors begin working at Good Karma-charismatic Samir (Harki Bhambra, Our Girl) and privileged Niki (Rebecca Ablack, Let It Snow)-each bringing their own perspectives to the team. Through it all, outspoken hospital head Lydia Fonseca (Amanda Redman, New Tricks) is forced to confront her complex past and decide her future when her partner, Greg (Neil Morrissey, Line of Duty), faces deportation.

Lydia is forced to admit a highly contagious Nipah patient into her hospital when Ruby and new British Asian doctor Samir break protocol in the fight to save the girl's life. Ram is livid when he discovers that new headstrong trainee doctor, Dr. Niki Sharma, has ignored him and broken patient confidentiality. Greg helps two holidaymakers, but his good deed lands him in trouble with the law.

Ruby faces difficult questions when her actions have devastating consequences. Bobby ends up as a reluctant patient at Good Karma and decides to take matters into his own hands, much to Frankie's dismay. Lydia hides how impressed she is with Samir but rewards his efforts with a job offer. Greg discovers that getting arrested has alerted immigration to the fact that he doesn't have a visa.

The happy news that Karishma is awake is offset by her loneliness. Realizing she needs more than medicine to fix her, Samir arranges a little magic. Niki volunteers to help Frankie find her missing son, and when they do, Niki discovers that empathy may be the best medicine, although Frankie's cancer worries require urgent tests. Meanwhile, Lydia learns there's only one answer to Greg's visa issue.
